Office Hours: Monday - Friday, 7:45 AM to 3:00 PM (No lunch break)
Application and Testing Fee: Php 2,000.00 (Non-refundable)
POLICIES ON ADMISSIONS
- All applicants to the High School are required to undergo the admissions screening process in order to provide the school with data to determine the eligibility of the applicant for acceptance in Xavier School.
- Parents will be informed about the result of the screening process. However, the details of the results will remain confidential.
- The applicant is given the admissions test once only. Re-testing is not allowed.
- All applicants should have passing grades in all subjects from the previous grade level, and in the First and Second Quarters of the current grade level.
- All accepted transferees are placed on probation status on their first year in the High School.
- The High School Unit accepts transferees in Grade 7 to 11 only, depending on the number of available slots.
- All applicants are required to undergo Vision and Hearing Screening Tests and a comprehensive medical check-up. Results of the tests should be submitted with the other requirements before testing. Note: The results of the vision and hearing screening tests may be deferred. Please note that if your child is deemed qualified for admission, he will be placed on conditional acceptance pending submission of all medical requirements.
- All accepted transferees are required to take the Hànyǔ Shuǐpíng Kǎoshì (HSK), the Chinese Proficiency Test once they are admitted to Xavier School.
GENERAL NORMS
- Xavier School’s High School Program will be beneficial to an applicant if he manifests:
- Positive self-discipline
- Good study habits
- Well-developed Interpersonal Skills
- Well-developed English, Filipino and Chinese Communication Skills
- Well-developed Stress and Time Management Skills
- Respect for the members of the school community
- The school also takes into account the following:
- The willingness of the parents of the student applicant to support and collaborate actively with the school in the education and formation of the student (which includes the parent’s acceptance of the school policies, especially found in the Student Handbook);
- The willingness of the applicant and his parents to be involved in important school activities (which includes off-campus activities that consist but not limited to field trips, service interactions, recollections and study abroad programs);
- The extent to which the applicant’s family environment is open to, and supportive of the Catholic Christian values espoused by the school.
- An applicant is evaluated primarily on his qualifications for admissions to High School.
- For applicants to Grade 7 and higher levels, the applicant must be eligible for the Chinese subject for the grade level they are applying for.
QUALIFICATIONS FOR ADMISSIONS TO HIGH SCHOOL
- High School Entrance Examination (HSEE) Results (which includes a Chinese test).
- Performance in the Current Grade Level
- An applicant with a failing grade in any subject by the end of March 2023 will NOT BE ACCEPTED to Xavier High School.
- Disciplinary Record
- An applicant who has been involved in any disciplinary case within the school year will NOT BE ACCEPTED to Xavier High School.
- Family Interview
- The interview will be scheduled in phases after the release of the results of the application for High School admission.
- The interview is a requirement for all applicants but is not a qualifying factor for admission.
- Accomplished Xavier School Application Form
- Learner’s Reference Number (LRN) from the current school
- Original copy of the Applicant’s Birth Certificate (PSA).
- Original copy of the report card of the previous grade level and photocopy of the grades in the First and Second Quarters/Trimester of the current grade level.
- Baptismal Certificate if baptized in the Roman Catholic Church (photocopy verified against the original)
- Accomplished Recommendation Form from the current school
- Three (3) copies of recent identical 1.5” X 1.5” photos in white background for the following:
- Application Form (1 copy)
- Test Permit (1 copy)
- Health Record (1 copy)
- Accomplished Student Medical Record
- Results of Vision and Hearing Screening Test.
- Accomplished Xavier School Personal Data Disclosure Terms of Agreement Form.
- Two (2) copies of recent identical family picture in size 4R (4"x6") - one (1) copy to be pasted on the application form and the second copy to be submitted upon acceptance to the school.
Note: Only immediate family members should be in the picture. The people in the picture should be wearing decent clothing and should NOT be wearing caps and sunglasses.
ADDITIONAL REQUIREMENTS FOR NATURALIZED FILIPINO AND NON-FILIPINO APPLICANTS
Filipino
- Naturalization Identification Certificate (NID), if naturalized Filipino, or Identification Certificate of Registration (ICR), if born abroad with Filipino Parents (Photocopy)
Non-Filipino (Permanent Resident)
- Alien Certification of Registration (ACR), (Photocopy)
- Immigration Certificate of Residence, (Photocopy)
Non-Filipino (Temporary Resident)
- Alien Certification of Registration (ACR), (Photocopy)
- Immigration Certificate of Residence, (Photocopy)
- Special Study Permit (SSP)
- Passport pages showing the following information:
- Passport Number
- Place Issued
- Date Issued
- Expiration Date
- Date of Entry into the Philippines
- Entry Status
- Length of Authorized Stay
Exempted from submitting of ACRs are the children of the following:
- Permanent Foreign residents
- Aliens with working permit
- Foreign Diplomat Personnel
- Personnel from duly accredited international organizations residing in the Philippines
- Holders of Special Investor's Resident Visa (SIRV)
- Holders of Special Retiree's Resident Visa (SRRV)
Note: Parents of the abovementioned students should submit to the Registrar's Office a copy of the documents supporting their residence status.
ADDITIONAL REQUIREMENTS FOR FOREIGN APPLICANTS
- An applicant coming from a foreign school whose medium of instruction is not English, must undergone at least one (1) school year of schooling in the Philippines prior to application to Xavier School.
- The final Report Card/Evaluation Report must be authenticated (Red Ribbon/Apostille) by the Philippine Embassy of the country of origin as part of the enrollment requirement.
MEDICAL REQUIREMENTS
- The applicant must have completed the following immunizations:
- DTaP : At least 5 doses. The last dose must be given on or after the 4th birthday.
- Tdap : 1 dose given after the 10th birthday
- MMR : 2 doses
- Varicella : 2 doses with last dose on or after the 4th birthday or verification of disease.
- Vision and Hearing Screening Test
- Go to the Xavier School Parents Portal at https://sa.xs.edu.ph/PARENTS_STUDENTS/parents_student_index.htm:
- Click on New User? Click here to Register.
- Choose Basic Ed (up to grade 10). Then click Proceed Next.
- Click TRANSFEREE then click Register
- Choose the appropriate Grade Level. The screen will automatically go to the next section.
- Kindly fill out completely all the General Student Personal Information Sheet (GSPIS). Then click on the button Click to Create Basic Information.
- Complete the System Login Information. Please take note of the Temporary Student Number and Password. You will need this information to access the Parents Portal for your transactions in Xavier School.
To print the application form, kindly do the following:
• log in again using your temporary ID and generated password
• Click on Personal Information
• then click on PRINT PERSONAL INFO located at the left side of the screen.
- Download the following forms by clicking on the file names:
- Student Health Record Form
- The Health Record Form should be filled out by your physician (and specialist for children with special medical conditions). Kindly print the form on legal-sized bond paper.
- Data Privacy Agreement Forms: The forms are to be signed by the parents to signify that they have read, understood and agree with the provisions of the Data Privacy Policy of the School. Kindly print the form on a short bond paper
- Recommendation Form
- Kindly print the form on legal-sized bond paper. The Recommendation Form should be filled out by the school head or teacher of your child’s current school. Please request the school head or teacher of your son to email the accomplished form to xssjadmissions@xs.edu.ph.
- Student Health Record Form
- Upload the scanned copies of the following documents by clicking on this link - Admissions Upload Facility:
- Application Form with the ID picture of the applicant and Family Picture attached, duly signed by the parents
- PSA Birth Certificate
- Report Card/Progress Report for the First Month/Quarter/Semester of the current school year
- Baptismal Certificate if baptized in the Roman Catholic Church (original and photocopy)
- Student Health Record Form
- Data Privacy Agreement Forms
- Instruction regarding the payment of the application and testing fees will be emailed to the parents of the applicant within five (5) working days upon verification of the application documents.
- The schedule of your son’s test will be sent to the email address you provided the school.
The test permit will be emailed to the parents of the applicant by the Testing and Research Center upon approval of the submitted documents. This includes the following:
- schedule of examination.,
- required gadgets for testing,
- important guidelines and procedures,
- link to access the online test, and
- link to submit the Reply Slip/Parental Consent Form.
Acknowledgement of the test permit is required.
- Applicants are expected to be at the testing area at least fifteen (15) minutes before the scheduled test. Latecomers will not be entertained.
- Testing may be rescheduled (and you will be given a new schedule via email) only in the following instances:
- The applicant is sick and not well enough to take the test.
- There is death in the family.
- Xavier School calls off classes/work due to inclement weather or other unforeseen events that could affect the scheduled online testing. We will contact you when this happens.
Note: For rescheduling and other testing concerns, please email the Testing and Research Center at trc-hsadmissions@xs.edu.ph.
- The applicant must strictly follow the test protocols during the onsite testing.
- Results of application will be emailed to the parents of the applicant.
- No results will be released by telephone.
ENROLLMENT RESERVATION
- For applicants who have been accepted, the parents are required to make a reservation to express their intention to enroll their son in Xavier School.
- If the reservation fee is not paid within ten (10) days after the release date of the test results, we will assume that you are no longer interested in your child’s admission to Xavier School. His application will be cancelled automatically.
- The acceptance letter will include instructions on how to make the reservation and how to pay the required Reservation Fee. For other concerns regarding the Reservation Fee, please feel free to see the School Treasurer.
ENROLLMENT FOR ACCEPTED APPLICANTS
The procedure for enrollment for accepted applicants will be announced before the end of the current school year.
GRANT-IN-AID PROGRAM
The Xavier School Educational and Trust Fund, Inc. (XSETF) was set up in 1966 to grant financial aid to deserving students, on the principle that “No student who is otherwise qualified, shall be
deprived of a Xavier education solely for financial reasons.” The Grant-in-Aid Program of the XSETF offers financially challenged but otherwise qualified students the opportunity of quality education that Xavier School offers.
Who are Qualified to Avail of the Grant-In-Aid (GIA) Program?
1. Applicant’s family cannot afford Xavier School education for financial reasons
2. Applicant who passes the admissions process of Xavier School
3. Applicants for Grades 2 to 5 and 7 to 11 must have Chinese studies (Mandarin) background
How does One Avail of the Grant/Scholarship?
1. The applicant who received an OUTRIGHT acceptance to Xavier School may secure the necessary forms from the XSETF Grant-In-Aid Office or download these at the website of the school.
2. The parents of the applicant will be interviewed regarding the child’s application for the grant.
3. Home Visit and/or In-Depth Interview will be conducted
What are the Conditions for the Approval of the Grant/Scholarship?
1. The applicant has received an OUTRIGHT acceptance to Xavier School after the admissions process.
2. The family has provided accurate data in the GIA application forms and submitted all the documentary requirements
3. The approved grant apply only for the specific school year applied for.
